BadgerCare Plus Members

Thank you for choosing Dean Health Plan as your HMO. As a member of Dean Health Plan you will receive all your health care from Dean Health Plan doctors, and hospitals. See our provider directory to choose a primary care provider or you may call our Customer Care Center at (800) 279-1301 for help choosing a provider.

Our Medicaid and BadgerCare plans are public health insurance programs developed by the State and Federal governments to assist the uninsured and under-insured children, families and pregnant women who meet specific income limits.

BadgerCare Plus Core Plan provides access to health care services to long-term uninsured adults, ages 19 through 64, who do not have children or do not have dependent children under age 19 living in the home.

Are you diabetic?

FamilyMake sure you are doing the things that help you stay healthy!

• Get a HbA1c test at least twice a year.
• Get a LDL-C test at least twice a year.
• Get a regular dilated eye exam.
• Get a regular foot exam.
• Eat a healthy diet.
• Exercise regularly.

Studies show that people who use tobacco products are more successful at quitting when both counseling and prescription help is used.

• Choose a doctor
• Schedule prenatal and postpartum doctor visits
• Connect with a nurse to answer your questions
• Find health education classes
• Make healthy changes, like quitting smoking
• Care for yourself and your baby before and after birth
• Get connected with local resources

Do you have a child under two years of age?

Keep your children healthy with the immunizations they need!

ImmunizationsBefore your child turns two they should receive the following shots:

• 4 DTap immunizations
• 3 Hib's
• 3 Polio vaccines
• 3 Hepatitis B vaccines
• 1 MMR
• 1 Varicella
• A blood lead test at one year of age
• A blood lead test at two years of age

All children should receive a HealthCheck at 15 and 18 months of age. It is at these HealthChecks they receive the fourth DTap shot.

Do you or your child have asthma?

Asthma ManagementManaging your asthma

Avoid these common asthma triggers or remove them from your home:

• Animal dander from pets. Keep animals out of the
  sleeping areas of the home at all times.
• Dust mites - wash bed sheets weekly in hot water.
• Pollen - keep windows closed from late morning to afternoon
  when pollen is highest.
• Smoke - do not smoke and do not allow people to smoke
  around you.
• Cold air - cover face on cold, windy days.

Set up an Asthma Action Plan with your doctor. This can help you know what to do if you have an asthma attack. Let your family and friends know the plan. Tell your doctor if you are having more symptoms.

Use controller medication every day as directed by your doctor. If you need to use the rescue inhaler more than 2 times per week, your asthma may not be well controlled.

See your doctor twice a year for asthma follow up.

Important Transportation Announcement

Starting July 1, 2011, the Wisconsin BadgerCare Plus program will provide transportation services to all Standard and Benchmark members no matter where you live. In the past, Dean Health Plan scheduled rides for members who lived in Dane County only. The BadgerCare Plus program has arranged with LogistiCare Solutions, LLC, to provide this service. You may be contacted by LogistiCare and/or be notified by the State of Wisconsin to let you know about this change.

After July 1, 2011, Dean Health Plan will no longer schedule cab rides. You will be referred to LogistiCare instead. You can contact LogistiCare at 1-866-907-1493, or for hearing impaired call 1-866 -288-3133. It is very important that you contact LogistiCare two days ahead of time to ensure transportation.

HealthCheck Program

HealthCheck is a preventive health checkup program for members up to age 21. The program covers the care for any health programs found during the checkup, including medical care, eye care and dental care.

Your doctor wants to see your children for regular checkups, not just when they are sick.

Comprehensive Physical Exam

Comprehensive ExamAs a member of the Core Plan, it is your responsibility to schedule and receive a physical exam with your doctor within the first year of being enrolled in the Core Plan. This is very important because if you do not get a physical exam, you will lose your health care benefits.
