Q. What does WIN stand for?
A. WIN stands for Wellness Incentives Now. It also means that it's a win-win situation: for you, your employer and Dean Health Plan.

Q. Who is eligible?
A. All Dean Health Plan commercial members are eligible to participate in the WIN program. This includes insured dependent children and spouses. DeanCare Rx and Medicaid members are not included in this program at this time.

Q. Will Dean reimbursement for Internet-based weight management programs?
A. Yes, Dean Health Plan will reimburse you for Weight Watchers and Jenny Craig Internet-based weight management programs. Other Internet-based weight management programs are not covered at this time.

Q. Can I submit a reimbursement for the food and meal cost of my weight loss program?
A. No, reimbursement does not include food, supplements or medication, but can be applied to membership dues or registration fees.

Q. When does my yearly $100 limit renew?
A. The program is based on a calendar year, January 1 through December 31. All reimbursement forms sent to Dean Health Plan for services received in this calendar year must be postmarked by December 31.

Q. How and when do I recieve reimbursement?
A. You pay for the service at the time it is rendered. Then, send in your receipt along with the Reimbursement Claim Form. Please send your receipts in on an 8½ x 11 sized piece of paper. Within 30 days we will send you a reimbursement check as a reward for your healthy endeavor! As you and your family receive eligible services, send in your form and receipts, and we will send your reimbursement until you reach the maximum.

Q. How much can I get back? How much can my family get back?
A. Dean will reimburse individual members up to $100 or families up to $200 for eligible services received per calendar year. Dean's Comprehensive Weight Management Program is a bonus $100 reimbursement and is not included in the $100 individual or $200 family maximum reimbursement limit.

Q. By what date do I need to turn in my reimbursement form?
A. The deadline for submitting a reimbursement form is December 31. Services are reimbursed for the year in which they were purchased.

Q. How does the additional $100 bonus reimbursement work?
A. By participating in the Dean Comprehensive Weight Management Program, members will receive a $100 bonus reimbursement. This does not count toward the individual or family WIN reimbursement maximum but is an additional incentive. Please note that members with insurance benefits, such as a weight management rider, that could be used to cover participation costs in Dean’s Comprehensive Weight Management Program are not eligible for the additional $100 reimbursement.

Q. What qualifies as proof of payment?
A. Proof of payment includes a credit card statement, bank statement, cancelled check or receipt. If you submit a credit card or bank statement, please indicate where the WIN-eligible charge appears.

Q. What is community supported agriculture?
A. Community supported agriculture (CSA) is a produce, dairy or meat membership. WIN reimburses for produce shares only. For more information CSA or to find a farm in your area, please visit www.macsac.org.
