Conversion Plan

The Conversion Plan is available to existing Dean Health Plan members who will be losing coverage through a group policy. Coverage is guaranteed; no medical review is required.

Eligibility

Your clients are eligible for this plan if the individual is:

• 18 years of age.
• Resides inside the Dean Health Plan service area.
• Losing coverage through a DHP Group Policy.

Coverage is guaranteed; no medical review is required.

If your clients reside outside the DHP service area, please contact our Customer Care Center to receive information about out-of-area conversion coverage.

If any applicant is eligible for Federal Medicare, please contact us for further information.

To Apply
In order to be eligible for this policy, an application must be received no later than 31 days following termination of Dean Health Plan coverage through their employer.
