Transfer Your Medical Records

Once you've chosen a doctor and called the clinic to make an appointment, have your medical records sent over from your previous clinic by filling out an Authorization to Release Protected Health Information form.

Sign Up for MyChart

Our patient portal allows you to:

• Securely and confidentially e-mail your doctor and clinic staff
• Request new appointments
• Review instructions and summary reports from recent clinic visits
• View lab results
• Request prescription renewals
• Set up MyChart accounts for your children
• And more 

We are also the first healthcare organization in the nation to offer a MyChart app for Apple® devices.
